You can register as a patient and make appointments online via our website. For this we process the following personal data:

• Date of birth
• BSN number (Dutch citizen service number)
• Address details
• Telephone number
• Email address
• Health insurance details
• Medical information you provide when registering or that is necessary for the appointment
  

Purposes of processing

We use these data to:

• Process your registration as a patient
• Contact you to schedule, change or confirm appointments
• Provide your treatment properly and safely
• Comply with legal obligations within healthcare
  

Legal basis for processing

We process your personal data because this is necessary for:

• Entering into and performing the treatment agreement
• Complying with legal obligations (such as the statutory retention of medical records)
• Your consent, when you voluntarily provide data via the registration form
  

Retention period

Your data are not retained longer than necessary. Medical records are retained in accordance with the statutory retention period of 20 years (or longer if medically necessary). Other administrative data are kept as long as legally required.

Sharing of data

Your data are only shared with parties directly involved in your treatment or in managing our practice administration, such as our practice management software provider. Processor agreements are in place with these parties to safeguard your privacy. Your data are never provided to third parties for commercial purposes.

Your rights

Under the GDPR you have the following rights:

• Right of access: you can request which personal data we process about you.
• Right to rectification: you can have incorrect data corrected.
• Right to erasure: you can request deletion of personal data (provided this does not conflict with statutory retention obligations).

• Right to data portability: you can request to receive your data in a structured, digital format.
