What personal data do we process?

Patient data

When you become or are a patient of Dovident, we may process the following personal data:

  • name, address and place of residence;
  • date of birth;
  • telephone number and email address;
  • citizen service number (BSN), where permitted or required by law;
  • health insurance information;
  • medical information relevant to your oral health and treatment;
  • treatment records, X-rays, scans, dental models and other information forming part of your patient record;
  • information relating to appointments, correspondence and payments.
Website data

When you visit our website, technical information may be processed, such as your IP address, browser information and information about your use of the website.

Why do we process personal data?

Providing dental care

Personal data is processed for maintaining patient records, making diagnoses, providing dental treatment and delivering aftercare.

Managing appointments

Personal data is processed for scheduling, changing and confirming appointments. Dovident may use email and SMS messages for this purpose.

Administration and invoicing

Personal data is processed for financial administration, invoicing and the collection of outstanding payments.

Legal obligations

Personal data is processed where necessary to comply with legal obligations applicable to healthcare providers.

Quality assurance and practice management

Personal data may be processed for quality assurance, practice management, complaint handling and the improvement of our services.

On what legal basis do we process personal data?

Dovident processes personal data on one or more of the following legal bases:

  • performance of a treatment agreement;
  • compliance with legal obligations;
  • legitimate interests, such as practice management, security and quality assurance;
  • consent, where legally required.

Who do we share personal data with?

Personal data is only shared where necessary for treatment, the provision of services or where Dovident is legally required to do so.

Depending on the situation, personal data may be shared with:

  • health insurance providers;
  • other healthcare professionals, such as dentists, dental hygienists, general practitioners, oral and maxillofacial surgeons and dental specialists;
  • dental laboratories;
  • providers of practice management software and IT services;
  • parties involved in invoicing and payment processing;
  • government authorities where required by law.

Personal data is not sold to third parties.

Where required by law, Dovident enters into data processing agreements with parties that process personal data on its behalf.

How long do we retain personal data?

Patient records

Patient records are subject to statutory retention periods. Under Dutch healthcare legislation, patient records are generally retained for a minimum of twenty years after the last amendment to the record.

Other personal data

Other personal data is not retained for longer than necessary for the purpose for which it was collected, unless a legal retention obligation applies.

Protection of personal data

Dovident implements technical and organisational measures to protect personal data against loss, unauthorised access and unlawful processing.

Access to personal data is restricted to persons who require access in order to perform their duties.

CCTV and Camera Use

CCTV in the reception area

CCTV is used in the reception and waiting area of the practice. The purpose of this CCTV system is to protect visitors, staff and property and to support the security of the practice.

CCTV recordings are retained for a maximum of four weeks. If an incident occurs, relevant recordings may be retained for a longer period where necessary for the handling of the incident or any investigation by authorised authorities.

Observation during X-ray examinations

During certain dental X-ray procedures, a camera is used in the X-ray room to allow staff to observe the patient remotely. This camera is used solely to monitor the patient during the procedure and to ensure safety during the examination.

The images are not recorded or stored.

Your rights

You have the right to:

  • request access to your personal data;
  • request correction of inaccurate personal data;
  • request completion of incomplete personal data;
  • request deletion of personal data in certain circumstances;
  • object to certain types of processing;
  • request restriction of processing;
  • request data portability, where legally applicable.

Legal exceptions may apply to medical records.

Complaints

If you have questions or complaints regarding the processing of your personal data, you may contact Dovident.

You also have the right to lodge a complaint with the Dutch Data Protection Authority (Autoriteit Persoonsgegevens) via www.autoriteitpersoonsgegevens.nl.
